Forms and Documents
This page lists the different forms or documents one might need from sponsoring a pilgrim to serving on a weekend. Feel free to download and use at your convenience.
Pilgrim Application Form:
This form is to be filled out by the new pilgrim and the sponsor to request a place in the next Emmaus Weekend. All the lines must be filled out by the pilgrim and the sponsor. Of particular importance for the pilgrim are the questions that pertain to their participation in a church, any health concerns that might impact their participation on a weekend, their understanding of the weekend as explained by the sponsor, and a brief description of their current Christian journey. The sponsor needs to be sure that the pilgrim has completed the application correctly and the sponsor must complete their own portion of the application, especially those questions that pertain to their 4th Day commitments, their pledge to participate in the Sponsors' Hour, Candlelight, Closing and assisting the new pilgrim in finding a 4th Day group.
Clergy Information Form:
All clergy of the TC Area Emmaus Community are requested to fill this form for the files of the Community Spiritual Director. The process is requested from the Upper Room, the owners of the copyright for The Walk to Emmaus. If you have any concerns whatsoever, feel free to contact the Community Spiritual Director.
YES! Form for Working on a Weekend:
If you have a desire to volunteer for a weekend, fill out this form and email it or send it to the addresses indicated. The Board takes these requests very seriously. Yes! forms allow the board to set up the weekend more quickly. If you are a new member of the community, fill this out so that we can get you involved right away with the upcoming walks, and you can see how important the volunteers are to the completion of a weekend.
Update Your Emmaus Information:
Much of our information on the members of our Emmaus community is out of date. Please fill out this form so that we can maintain communication with you concerning the life and ministry of the TC Area Emmaus Community. Most importantly, we need an updated mailing address, telephone number and email address. If you want to receive the quarterly newsletter, be sure to fill out this form.
Weekend Speaker Request Form:
All volunteers who are giving talks for the weekend MUST fill out this form. Completed, it will contain information essential for the flow of the weekend in the conference room.
Weekend Team Registration Form:
This MUST be fill out by all members volunteers of the weekend. The kitchen need to know if you are staying for meals so they can make their plans, and Auxiliary needs to know if you are sleeping at the church for the weekend so they can make arrangements for your bed. All member volunteers MUST also send in a check with their form that will cover the cost of their food. Member weekend volunteers need to fill this out early in the plannning and training process. Please, do not wait until the Thursday of the weekend to fill out the form and pay for your food. The kitchen volunteers need your contribution to help in the purchase of food for the weekend.
